Разработка подхода к снижению частоты внеплановой реваскуляризации коронарных и периферических артерий путем воздействия на модифицируемые факторы риска.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Когорта пациентов с ишемической болезнью сердца (n=218; 63±10,9 лет, 54% мужчины), которым была выполнена коронарография (КАГ), в т.ч. со стентированием. У всех пациентов анализировали ряд рутинных антропометрических и биохимических параметров. Статистический анализ был выполнен с помощью программного обеспечения Statistica версии 8.0 и SPSS IBM статистика 23 с применением ROC-анализа, биномиальной логистической регрессии, таблиц сопряженности 2´2 и c2 Пирсона для проверки гипотезы.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена комбинация неинвазивных параметров, ассоциированная с максимальным значением относительного риска (relative risk, RR) реваскуляризации коронарных и периферических артерий в течение следующих 3 лет после КАГ (RR=15 (95% доверительный интервал (ДИ): 5,96-36,5, p=0,001), по сравнению с другими возможными комбинациями исследованных параметров. Данную совокупность составили окружность талии с границей 89 см, статус курения и четыре биохимических маркера: С-реактивный белок с границей 3,4 мг/л, глюкоза с границей 7,6 ммоль/л, инсулин с границей 10 мкЕд/мл и холестерин липопротеинов низкой плотности с границей 2,9 ммоль/л.</p></sec><sec><title>Заключение</title><p>Заключение. Была разработана шкала-таблица относительного риска (RR) реваскуляризации как коронарного, так и периферического кровотока в течение следующих 3 лет после плановой диагностической КАГ. Шкала-таблица демонстрирует возможность управления риском реваскуляризации коронарных и периферических артерий после КАГ и мотивировать пациента к управлению модифицируемыми факторами риска.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To develop an approach to reduce the frequency of urgent coronary and peripheral revascularization by influencing modifiable risk factors.</p></sec><sec><title>Material and methods</title><p>Material and methods. A cohort of patients with coronary artery disease (n=218; 63±10,9 years, 54% men) who underwent coronary angiography (CAG), including stenting. A number of routine anthropometric and biochemical parameters were analyzed in all patients. Statistical analysis was performed using Statistica version 8.0 and SPSS IBM statistics 23 software using ROC analysis, binomial logistic regression, 2´2 contingency tables and Pearson c2 for hypothesis testing.</p></sec><sec><title>Results</title><p>Results. A combination of noninvasive parameters associated with the maximum relative risk (RR) of coronary and peripheral artery revascularization over the 3 years after CAG was identified (RR=15 (95% confidence interval (CI): 5,96-36,5, p=0,001), compared with other possible combinations of the studied parameters. This combination included waist circumference with a limit of 89 cm, smoking status, and four following biochemical markers: C-reactive protein with a limit of 3,4 mg/l, glucose with a limit of 7,6 mmol/l, insulin with a limit of 10 μU/ml, and low-density lipoprotein cholesterol with a limit of 2,9 mmol/l.</p></sec><sec><title>Conclusion</title><p>Conclusion. A scale-table of the relative risk (RR) of coronary and peripheral revascularization over the 3 years after elective diagnostic CAG.
