Изучить изменения гемодинамики по данным трансторакальной эхокардиографии, течение хронической сердечной недостаточности (ХСН), качество жизни (КЖ) пациентов, осложнения и летальность в течение 12 мес. после имплантации устройства механической поддержки левого желудочка (LVAD).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 53 пациента с терминальной ХСН, соответствующих критериям имплантации LVAD. Группу 1 составили 23 пациента, которым проведена имплантация, в группу 2 вошли 30 пациентов, отказавшихся от операции. Исходно, перед выпиской, через 3, 6, 12 мес. проводились: трансторакальная эхокардиография, тест 6-минутной ходьбы, оценка функционального класса (ФК) ХСН, КЖ, концентрации N-концевого промозгового натрийуретического пептида, количества госпитализаций, осложнений и летальных исходов.</p></sec><sec><title>Результаты</title><p>Результаты. Летальность в группе 1 составила 30,4%, в группе 2 — 30,0%. Были зарегистрированы следующие осложнения имплантации LVAD: правожелудочковая недостаточность — 13, желудочно-кишечное кровотечение — 3, желудочковая тахикардия — 4, LVAD-ассоциированная инфекция — 14 случаев. В отличие от группы 2 в группе 1 уменьшились конечно-диастолический размер левого желудочка с 7,4 [7,1; 8,0] до 6,4 [5,7; 6,9] см (р&lt;0,001), митральная регургитация с 2,2±0,54 до 1,44±0,4 ст. (р&lt;0,001), концентрация N-концевого фрагмента предшественника мозгового натрийуретического пептида с 2239 [1623; 4057] до 740 [438,5; 1273] пг/мл (р&lt;0,001), улучшился ФК ХСН (p&lt;0,001), зарегистрировано меньшее количество госпитализаций — 23 vs 37 (р=0,014).</p></sec><sec><title>Заключение</title><p>Заключение. В настоящем исследовании имплантация LVAD привела к улучшению внутрисердечной гемодинамики, ФК ХСН, КЖ, уменьшению количества госпитализаций, декомпенсаций ХСН, но не снижению летальности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study hemodynamic changes according to transthoracic echocardiography, heart failure (HF) course, quality of life (QOL) of patients, complications and mortality within 12 months after implantation of a left ventricular assist device (LVAD).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 53 patients with endstage HF who met the criteria for LVAD implantation. Group 1 included 23 patients who underwent implantation, while group 2 included 30 patients who refused the intervention. At baseline, before discharge, and after 3, 6, and 12 months, the following were performed: transthoracic echocardiography, 6-minute walk test, assessment of HF class, quality of life, N-terminal pro-brain natriuretic peptide, hospitalization rate, complications, and fatal outcomes.</p></sec><sec><title>Results</title><p>Results. Mortality in group 1 was 30,4%, and in group 2 — 30,0%. The following complications of LVAD implantation were recorded: right ventricular failure — 13, gastrointestinal bleeding — 3, ventricular tachycardia — 4, LVAD-associated infection — 14 cases. Unlike group 2, in group 1, we recorded decrease of LV end-diastolic dimension from 7,4 [7,1; 8,0] to 6,4 [5,7; 6,9] cm (p&lt;0,001), mitral regurgitation from 2,2±0,54 to 1,44±0,4 st, (p&lt;0,001), N-terminal pro-brain natriuretic peptide from 2239 [1623; 4057] to 740 [438.5; 1273] pg/ml (p&lt;0,001), as well as improvement of HF class (p&lt;0,001), and lower hospitalization rate — 23 vs 37 (p=0,014).</p></sec><sec><title>Conclusion</title><p>Conclusion.
