Оценить связь распределения абдоминальной жировой ткани (ЖТ) с метаболической моделью инсулинорезистентности (ИР) — Mets-IR, в зависимости от наличия метаболического синдрома (МС) у пациентов со стабильной ишемической болезнью сердца (ИБС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. 61 пациент с хронической ИБС. Значимость коронарного атеросклероза определяли по индексу Gensini Score. МС определяли согласно критериям Международной федерации диабета (2005 г). Выполняли магнитно-резонансную томографию брюшной полости на уровне L4-L5 для определения объема абдоминальных жировых депо. Определяли в крови уровни глюкозы и инсулина, показателей липидного профиля, адипокинов. Резистентность к инсулину определяли путем расчета индексов гомеостатической модели ИР (НОМА-IR) и Mets-I R.</p></sec><sec><title>Результаты</title><p>Результаты. Группа с МС имела статистически значимо бóльшие индексы массы тела и окружность талии, выявлены межгрупповые различия в уровнях глюкозы, сывороточного лептина и Mets-I R. В группе с МС абдоминальные подкожная (ПЖТ) и висцеральная (ВЖТ) ЖТ показали сильную корреляционную связь с Mets-I R. Пациенты с выраженной ИР характеризовались повышенными объемами ВЖТ и ПЖТ, разницы в отношении ВЖТ/ПЖТ не выявлено. Индекс массы тела, окружность талии, как и уровень глюкозы, триглицеридов, С-реактивный белок, определенный высокочувствительным методом, в сыворотке крови были статистически значимо выше, а уровень холестерина липопротеинов высокой плотности ниже при большей выраженности ИР.</p></sec><sec><title>Заключение</title><p>Заключение. Связь ИР с объемом ЖТ в брюшной полости варьирует в зависимости от степени выраженности ИР и различается в зависимости от наличия или отсутствия МС. Среди лиц с хронической ИБС абдоминальное висцеральное ожирение ассоциируется с МС. Применение Mets-IR продемонстрировало свою валидность в оценке ИР у пациентов с ИБС и МС, сильную ассоциативную связь с показателями магнитно-резонансной томографии объема абдоминальных жировых депо.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the association between abdominal fat distribution and metabolic score of insulin resistance (IR) — Mets-IR, depending on the presence of metabolic syndrome (MS) in patients with stable coronary artery disease (CAD).</p></sec><sec><title>Material and methods</title><p>Material and methods. Sixty-one patients with chronic CAD. The significance of coronary atherosclerosis was determined using the Gensini Score. MS was defined according to the International Diabetes Federation criteria (2005). Abdominal magnetic resonance imaging (MRI) was performed at the L4-L5 level to determine the volume of abdominal fat depots. Blood glucose and insulin levels, lipid profile parameters, and adipokines were determined. Insulin resistance was assessed by calculating the homeostatic model assessment of IR (HOMA-IR) and Mets-I R.</p></sec><sec><title>Results</title><p>Results. The MS group had significantly higher body mass indices and waist circumference, and between-group differences were found in glucose, serum leptin, and Mets-IR levels. In the MS group, abdominal subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) showed a strong correlation with Mets-I R. Patients with severe IR were characterized by increased VAT and SAT volumes; no difference in the VAT/SAT ratio was found. Body mass index, waist circumference, serum glucose, triglyceride, and high-sensitivity C-reactive protein levels were significantly higher, while high-density lipoprotein cholesterol levels were lower with greater IR severity.</p></sec><sec><title>Conclusion</title><p>Conclusion. The relationship between IR and abdominal adipose tissue volume varies depending on the severity of IR and differs depending on the presence of metabolic syndrome. Among individuals with chronic coronary artery disease (CAD), abdominal visceral obesity is associated with metabolic syndrome.
