Очевидно, что с повышением качества и увеличением продолжительности жизни населения в клинической практике все чаще будут встречаться коморбидные пациенты с критическим АС, у которых основным методом хирургического лечения будет ТИАК. Несмотря на достигнутые успехи в ведении пациентов с сердечно-сосудистыми заболеваниями, их непрерывное прогрессирование может привести к развитию de novo атеросклеротического поражения или рестеноза в ранее стентированном артериальном сегменте коронарного русла. Конструкция протезов аортального клапана, используемых при ТИАК, может наложить ряд технических ограничений при необходимости повторных интервенций на коронарных артериях. В настоящей статье представлен клинический случай успешного эндоваскулярного лечения многососудистого поражения коронарного русла под контролем внутрисосудистого определения показателей коронарного кровотока у коморбидной пациентки, ранее перенесшей ТИАК.</p></abstract><trans-abstract xml:lang="en"><p>Transcatheter aortic valve implantation (TAVI) is an effective and safe method for treating aortic stenosis (AS), especially in patients at high surgical risk. With the improvement of the quality and increase in life expectancy of the population, comorbid patients with critical AS will increasingly be encountered in practice, for whom TAVI will be the main method of surgical treatment. Despite the achieved successes in the management of patients with cardiovascular diseases, their cotinuous progression can lead to de novo atherosclerotic lesions or restenosis in the previously stented coronary segment. The design of prosthetic aortic valves used in TAVI can impose a number of technical limitations when repeated coronary interventions are necessary.
