Оценить роль параметров стресс-эхокардиографии (СЭ) по протоколу ABCDE как предикторов сердечно-сосудистых событий (ССС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В одноцентровое проспективное исследование включено 253 пациента (мужчин n=130, 51,4%), медиана возраста 62 (53; 68) года с подтвержденной (14,2%) и подозреваемой ишемической болезнью сердца. СЭ с физической нагрузкой проводилась по клиническим показаниям. Протокол ABCDE включал оценку нарушений локальной сократимости (шаг А), В-линий (шаг В), сократительного (шаг С), коронарного (шаг D), хронотропного (шаг Е) резервов левого желудочка. Медиана срока наблюдения 198 (118; 324) дней, у 21 (8,3%) пациента выполнена реваскуляризация, 11 пациентов (4,4%) повторно госпитализированы по поводу ССС. Оценивалось влияние клинических и эхокардиографических показателей на вероятность развития конечных точек.</p></sec><sec><title>Результаты</title><p>Результаты. Частота аномальных результатов СЭ составила 8,7% (n=22) для шага А, 10,7% (n=27) для шага В, 63,6% (n=161) для шага С, 47,4% (n=120) для шага D и 35,6% (n=90) для шага E. Независимым предиктором повторной госпитализации был профиль (комбинация положительных шагов по результатам СЭ) ACDE (отношение шансов — odds ratio, OR 22,4; р=0,003); предикторами реваскуляризации — сумма баллов ABCDE (OR) 24,4; р=0,005), характер боли в груди (OR 7,2; р=0,005), профиль DE (OR 762; р=0,003), величина хронотропного резерва (OR 437; р=0,019), количество факторов риска (OR 7,7; р=0,009).</p></sec><sec><title>Заключение</title><p>Заключение. СЭ по протоколу ABCDE позволяет провести комплексную оценку функционального состояния сердца, улучшить диагностическую способность стресс-теста и выявить предикторы ССС у пациентов с подозреваемой и подтвержденной ишемической болезнью сердца.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the role of stress echocardiography (SE) parameters according to ABCDE protocol as predictors of cardiovascular events (CVD).</p></sec><sec><title>Material and methods</title><p>Material and methods. This single-center prospective study included 253 patients (men n=130, 51,4%; median age, 62 (53; 68) years) with confirmed (14,2%) and suspected coronary artery disease (CAD). Exercise SE was performed according to clinical indications. The ABCDE protocol included assessment of regional wall motion (step A), B-lines (step B), contractile (step C), coronary (step D), chronotropic (step E) reserves of the left ventricle. The median follow-up period was 198 (118; 324) days. Twenty one (8,3%) patients underwent revascularization, while 11 patients (4,4%) were rehospitalized for cardiovascular events. The influence of clinical and echocardiographic parameters on endpoints was assessed.</p></sec><sec><title>Results</title><p>Results. The incidence of abnormal SE results was 8,7% (n=22) for step A, 10,7% (n=27) for step B, 63,6% (n=161) for step C, 47,4% n=120) for step D, and 35,6% (n=90) for step E. An independent predictor of readmission was the ACDE profile (a combination of positive SE steps) (odds ratio, OR 22,4; p=0,003); predictors of revascularization were the ABCDE score (OR 24,4; p=0,005), chest pain pattern (OR 7,2; p=0,005), DE profile (OR 762; p=0,003), chronotropic reserve (OR 437; p=0,019), number of risk factors (OR 7,7; p=0,009).</p></sec><sec><title>Conclusion</title><p>Conclusion.
