Оценить клинико-гемодинамические, лабораторные и инструментальные характеристики пациентов с инфарктом миокарда (ИМ), у которых в течение 48 ч после поступления развился кардиогенный шок (КШ) стадии B и выше при отсутствии признаков шока на момент госпитализации, и определить факторы, ассоциированные с летальным исходом.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В ретроспективный анализ включены 60 пациентов. В качестве основной конечной точки рассматривалась смерть в стационаре. Применялся одно- и многофакторный логистический регрессионный анализ.</p></sec><sec><title>Результаты</title><p>Результаты. Летальность среди данной когорты пациентов составила 55%. Независимыми предикторами летального исхода в многофакторной модели явились: наличие сахарного диабета 2 типа (отношение шансов (OR — odds ratio) 149,6; p=0,001) и более высокое систолическое артериальное давление при поступлении (OR 1,08 за каждый 1 мм рт.ст.; p=0,015). Кроме того, достижение оптимальной реперфузии (TIMI 3 — Thrombolysis In Myocardial Infarction) оказалось независимым протективным фактором (OR 0,009; p=0,015). Площадь под ROC-кривой модели составила 0,70.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с ИМ и последующим развитием КШ уже при поступлении могут выявляться признаки, соответствующие стадии A по классификации SCAI (Society for Cardiovascular Angiography and Interventions). Эти параметры обладают прогностической значимостью и могут использоваться для ранней стратификации риска летального исхода и выбора стратегии мониторинга.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the clinical, hemodynamic, and paraclinical characteristics of patients with myocardial infarction (MI) who developed stage B or higher cardiogenic shock (CS) within 48 hours of admission in the absence of shock signs at admission, and to identify factors associated with mortality.</p></sec><sec><title>Material and methods</title><p>Material and methods. This retrospective analysis included 60 patients. The primary endpoint was inhospital death. Univariable and multivariable logistic regression analyses were used.</p></sec><sec><title>Results</title><p>Results. Mortality in this cohort of patients was 55%. Independent predictors of mortality in the multivariable model were type 2 diabetes (odds ratio (OR) 149,6; p=0,001) and higher systolic blood pressure on admission (OR 1,08 per 1 mm Hg; p=0,015). Furthermore, achieving optimal reperfusion (Thrombolysis In Myocardial Infarction (TIMI) 3 was an independent protective factor (OR 0,009; p=0,015). The area under the ROC curve for the model was 0,70.</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients with MI followed by CS, signs of Society for Cardiovascular Angiography and Interventions (SCAI) stage A may be detected already upon admission.
