Частота осложнений эндоваскулярного закрытия открытого овального окна (ООО) считается невысокой, но для большинства из них она точно не установлена. Наиболее серьезными интраоперационными осложнениями являются повреждение стенки сердца и сосудов с развитием кровотечения, неправильное расположение окклюдера, тромбоэмболия, воздушная эмболия. Сообщения о тяжелых и потенциально летальных осложнениях единичны, в связи с чем их рассмотрение, а также определение тактики лечения является актуальным.</p></sec><sec><title>Краткое описание</title><p>Краткое описание. Пациентка К., 36 лет, перенесла ишемический инсульт. При обследовании выявлено ООО со значительным шунтированием. С целью профилактики повторных ишемических событий рекомендовано эндоваскулярное закрытие ООО. Во время операции зарегистрирован подъем сегмента ST в отведениях II и III. На селективной коронарной ангиографии: дистальные отделы заднебоковой и задней нисходящей артерии не контрастируются. Интракоронарно введен нитроглицерин, что привело к восстановлению кровотока в артерии. После стабилизации состояния пациентки эндоваскулярное закрытие ООО было успешно завершено, окклюдер имплантирован.</p></sec><sec><title>Дискуссия</title><p>Дискуссия. Возможными причинами интраоперационного подъема сегмента ST являются воздушная эмболия пузырьками воздуха, оставшимися в системе доставки после промывания, механическое раздражение межпредсердной перегородки. Меры профилактики включают тщательное промывание системы доставки и выбор размера окклюдера в соответствии с анатомическими особенностями сердца.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The complication rate of endovascular patent foramen ovale (PFO) closure is considered low. However, for most of complications, the rate has not been precisely determined. The most serious intraoperative complications include damage to the heart wall and vessels with bleeding, malpositioned occlude device, thromboembolism, and air embolism. Reports of severe and potentially fatal complications are rare, making their consideration and the determination of treatment strategies crucial.</p></sec><sec><title>Brief description</title><p>Brief description. Patient K. aged 36 years had an ischemic stroke. Examination revealed a PFO with significant bypass grafting. Endovascular closure of the PFO was recommended to prevent recurrent ischemic events. During surgery, ST segment elevation was recorded in leads II and III. Selective coronary angiography revealed no contrast enhancement in the distal posterolateral and posterior descending arteries. Intracoronary nitroglycerin was administered, which restored flow in the artery. After the patient's condition stabilized, endovascular closure of the PFO was successfully completed, and an occluder was implanted.</p></sec><sec><title>Discussion</title><p>Discussion. Possible causes of intraoperative ST segment elevation include gas embolism from air bubbles remaining in the delivery system after flushing, and mechanical irritation of the interatrial septum.
