С использованием биоресурсной коллекции исследовать экспрессию цитокинов, хемокинов и факторов роста у пациентов с инфарктом миокарда (ИМ) &gt;80 лет и установить их взаимосвязь с развитием неблагоприятных исходов.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В ретроспективное исследование включены 59 пациентов &gt;80 лет с острым коронарным синдромом (ОКС). Анализировались клинико-демографические данные, лабораторные показатели, динамика биомаркеров при поступлении в стационар и 7 сут. Оценивались 30-дневные исходы. В зависимости от исходов пациенты были разделены на группы: группа 1 (n=11) – пациенты, умершие в течение 30 дней после ОКС, группа 2 (n=48) – остальные пациенты.</p></sec><sec><title>Результаты</title><p>Результаты. Динамическое наблюдение в общей группе показало значимое снижение сывороточных уровней интерлейкинов (ИЛ)-6, ИЛ-8, гранулоцитарного колониестимулирующего фактора на 7 сут. после ОКС, а содержание эотаксина, напротив, увеличивалось. Выявлены ассоциации провоспалительных цитокинов с показателями клинических шкал стратификации риска. В группе с летальным исходом отмечены более высокие уровни ИЛ-10, ИЛ-15, фактора некроза опухоли альфа, моноцитарного хемотаксического белка 1, гранулоцитарного колониестимулирующего фактора, макрофагального колониестимулирующего фактора и N-концевого промозгового натрийуретического пептида на 7 сут. ОКС. Сочетание N-концевого промозгового натрийуретического пептида &gt;1000 пг/мл и ИЛ-15 &gt;25 пг/мл на 7 сут. обладает наибольшей прогностической ценностью для оценки прогноза смерти пациентов в течение 30 сут. после ОКС с чувствительностью 83,3% и специфичностью 90,6% (площадь под ROC-кривой=0,87).</p></sec><sec><title>Заключение</title><p>Заключение. С использованием биоресурсной коллекции и мультиплексной технологии исследована экспрессия цитокинов, хемокинов и факторов роста у пациентов с ИМ &gt;80 лет. Разработана модель, позволяющая прогнозировать развитие неблагоприятных событий у лиц с ИМ.</p></sec><sec><title>Ключевые слова</title><p>Ключевые слова: острый коронарный синдром, инфаркт миокарда, пожилые пациенты, старческий возраст, цитокины, хемокины, факторы роста, биоресурсная коллекция.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Using a bioresource collection, to study the expression of cyto­kines, chemokines, and growth factors in patients with myocardial in­farc­tion (MI) aged 80 years and older and to establish their rela­tionship with unfavorable outcomes.</p></sec><sec><title>Material and methods</title><p>Material and methods. This retrospective study included 59 patients aged 80 years and older with acute coronary syndrome (ACS). Cli­ni­cal and demographic data, laboratory parameters, and biomarker dynamics at hospital admission and after 7 days were analyzed. Thirty-day outcomes were assessed. Patients were divided into groups based on outcomes as follows: group 1 (n=11) — patients who died within 30 days after ACS; group 2 (n=48) — the remaining patients.</p></sec><sec><title>Results</title><p>Results. Follow-up in the overall group revealed a significant decrease in serum levels of interleukin (IL)-6, IL-8, and granulocyte colony-­sti­mulating factor at day 7 after ACS, while eotaxin levels, conversely, in­creased. Associations between proinflammatory cytokines and clinical risk stratification scores were revealed. In the fatal group, higher levels of IL-10, IL-15, tumor necrosis factor-­alpha, monocyte chemotactic protein 1, granulocyte colony-­stimulating factor, macrophage colony-sti­mulating factor, and N-terminal pro-brain natriuretic peptide (NTproBNP) were noted at day 7 after ACS. The combination of NTproBNP &gt;1000 pg/ml and IL-15 &gt;25 pg/ml on day 7 has the highest prognostic value for predicting patient mortality within 30 days after ACS, with a sensitivity of 83,3% and a specificity of 90,6% (AUC=0,87).</p></sec><sec><title>Conclusion</title><p>Conclusion. Using a bioresource collection and multiplex technology, the expression of cytokines, chemokines, and growth factors was stu­died in patients over 80 years of age with myocardial infarction.
