Продемонстрировать пути создания и перспективы применения биомедицинских ресурсных коллекций биоматериала пациентов детского возраста, а также возможности их дифференцированного использования в клинических и научных целях в условиях многопрофильного стационара на базе научно-­исследовательского центра федерального уровня.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В работу был включен биоматериал пациентов детского возраста, а также их родственников первой линии родства, забанкированный в период с 2016 по 2025гг в рамках реализации научных проектов НМИЦ им. В. А. Алмазова Минздрава России в области клинической генетики и педиатрии. Выделенная из образцов цельной крови и сухих пятен крови дезоксирибонуклеиновая кислота (ДНК) использовалась для генетического тестирования с применением современных методов молекулярно-­генетической диагностики.</p></sec><sec><title>Результаты</title><p>Результаты. Возможности применения биоресурсных коллекций биоматериала пациентов детского возраста продемонстрированы на примере семейных случаев редких моногенных заболеваний, для которых ранее собранный биоматериал биобанка был использован для постановки диагноза и определения тактики дальнейшего наблюдения членов семьи, определения рисков для будущего потомства, углубления знаний по ведению пациентов с конкретным наследственным синдромом. В дополнение — возможность предполагать наличие новых фенотипов, ассоциированных с известным геном, и наоборот, раскрывать связь новых генов с известным фенотипом продемонстрирована на примерах семейных заболеваний, где биоматериал биобанка обеспечил возможность проведения сегрегационного анализа. На примере биоматериала когорт пациентов с определенной патологией проиллюстрирована возможность создания новых диагностических алгоритмов и лабораторных тест-систем, дифференцированного подхода к выбору таргетной генетической панели для диагностики, а также выявления новых клинико-­генетических особенностей заболевания. Показана возможность определения мутации основателя на примере патологии, выявленной в конкретной этнической группе с потенциальной возможностью описания времени ее возникновения и особенностей географического распространения.</p></sec><sec><title>Заключение</title><p>Заключение. Создание биоресурсных коллекций, полученных от детского контингента, является сложной, но крайне важной, клинически необходимой и перспективной научной задачей. Ее реализация способствует проведению трансляционных научно-исследовательских работ как для усовершенствования методов диагностики, понимания патогенеза, особенностей течения заболеваний детского возраста, влияния факторов окружающей среды на их формирование, поиска новых генетических причин развития наследственной патологии и описания новых фенотипов, ассоциированных с известными генами, выявления предикторов тяжести течения и прогноза, так и в долгосрочной перспективе — для поиска новых терапевтических подходов и понимания механизмов развития заболеваний у взрослых.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To demonstrate the development and potential applications of bio­medical resource collections of pediatric patient biomaterials, as well as the potential for their differentiated use for clinical and research pur­poses in a multidisciplinary hospital setting at a federal research center.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included biomaterial from pediatric pa­tients and their first-­degree relatives, banked between 2016 and 2025 as part of research projects at the Almazov National Medical Re­search Center in the field of clinical genetics and pediatrics. De­oxy­ri­bo­nucleic acid (DNA) isolated from whole blood samples and dried blood spots was used for genetic testing using modern molecular genetic dia­gnos­tic methods.</p></sec><sec><title>Results</title><p>Results. The potential of using bioresource collections of pediatric pa­tient biomaterial was demonstrated using familial cases of rare mo­no­genic diseases, for which previously collected biomaterial from the biobank was used to establish a diagnosis and determine follow-up strategies for family members, identify risks to future offspring, and re­ceive advance data on the management of patients with spe­ci­fic he­re­ditary syndromes. Additionally, the ability to predict new phe­no­ty­pes associated with a known gene, and conversely, to uncover the rela­tion­ship of new genes with a known phenotype, was demonstrated using examp­les of familial diseases, where biobank biomaterial enabled seg­re­gation analysis. Using biomaterial from cohorts of patients with a specific pathology, the potential for developing novel diagnostic al­go­rithms and laboratory test systems, a differentiated approach to se­lec­ting a targeted genetic panel for diagnosis, and the identification of new clinical and genetic disease features was showed. A founder mu­ta­tion was showed to be identified in a pathology determined in a spe­cific ethnic group, with the potential to describe the onset time and geo­graphic distribution.</p></sec><sec><title>Conclusion</title><p>Conclusion. The creation of bioresource collections obtained from pediatric populations is a complex, yet extremely important, clini­cal­ly necessary, and promising scientific task. Its implementation faci­li­tates translational research aimed at improving diagnostic methods, under­stan­ding the pathogenesis and progression of childhood diseases, and the influence of environmental factors on their development. It also faci­litates the search for new genetic causes of hereditary pathologies and the description of new phenotypes associated with known genes. It also identifies predictors of disease severity and prognosis.
