Оценить диагностические возможности магнитно-резонансной томографии (МРТ) сердца с использованием параметров деформации миокарда левого желудочка (ЛЖ) в дифференциальной диагностике необструктивной гипертрофической кардиомиопатии (ГКМП) и амилоидоза сердца.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В одноцентровое ретроспективное обсервационное исследование включены 48 пациентов: 23 с необструктивной ГКМП и 25 с амилоидозом сердца (ALи ATTR-типы). Всем пациентам выполнена МРТ сердца с контрастированием. Оценивали морфофункциональные параметры ЛЖ, паттерны отсроченного контрастирования и показатели глобальной и региональной деформации миокарда (2D и 3D strain). Статистический анализ включал сравнение групп и ROC-анализ для определения диагностической значимости strain-параметров.</p></sec><sec><title>Результаты</title><p>Результаты. Атипичные паттерны отсроченного контрастирования или его отсутствие выявлены у 22% пациентов, что затрудняло визуальную дифференциацию между ГКМП и амилоидной кардиомиопатией. Среди параметров деформации миокарда наиболее значимые различия между группами выявлены для радиальной 3D-деформации ЛЖ, которая была достоверно ниже у пациентов с амилоидной кардиомиопатией по сравнению с ГКМП (30,00±17,92 vs 45,98±20,16%; p=0,003). Пороговое значение 3D radial strain &lt;29,1% обеспечивало чувствительность 87% и специфичность 57% в дифференциальной диагностике данных заболеваний. Различия по показателям продольной и циркулярной деформации не достигли статистической значимости.</p></sec><sec><title>Заключение</title><p>Заключение. Параметры деформации миокарда, в частности радиальная 3D-деформация ЛЖ, обладают высокой диагностической ценностью при дифференциальной диагностике необструктивной ГКМП и кардиального амилоидоза, особенно в случаях атипичных или отсутствующих паттернов отсроченного контрастирования. Анализ деформации миокарда может рассматриваться как перспективный бесконтрастный метод, расширяющий диагностические возможности МРТ сердца и создающий предпосылки для внедрения коротких скрининговых МР-протоколов у пациентов с гипертрофией миокарда неясного генеза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the diagnostic potential of cardiac magnetic resonance imaging (MRI) using left ventricular (LV) myocardial strain parameters in the differential diagnosis of non-obstructive hypertrophic cardiomyopathy (HCM) and cardiac amyloidosis.</p></sec><sec><title>Material and methods</title><p>Material and methods. This single-center, retrospective, observational study included 48 patients as follows: 23 with non-obstructive HCM and 25 with cardiac amyloidosis (AL and ATTR types). All patients underwent contrast-enhanced cardiac MRI. Left ventricular morphofunctional parameters, delayed enhancement patterns, and global and regional myocardial strain indices (2D and 3D strain) were assessed. Statistical analysis included group comparisons and ROC analysis to determine the diagnostic significance of strain parameters.</p></sec><sec><title>Results</title><p>Results. Atypical patterns of delayed enhancement or its absence were detected in 22% of patients, complicating imaging differentiation between HCM and amyloid cardiomyopathy. Among myocardial strain parameters, the most significant differences between groups were found for 3D radial strain of the LV, which was significantly lower in patients with amyloid cardiomyopathy compared to HCM (30,00±17,92 vs 45,98±20,16%; p=0,003). A 3D radial strain cutoff value &lt;29,1% provided a sensitivity of 87% and a specificity of 57% in the differential diagnosis of these diseases. Differences in longitudinal and circumferential strain did not reach statistical significance.</p></sec><sec><title>Conclusion</title><p>Conclusion. Myocardial strain parameters, particularly LV 3D radial strain, have high diagnostic value in the differential diagnosis of non-obstructive HCM and cardiac amyloidosis, especially in cases of atypical or absent delayed enhancement patterns.
