Данных о возможностях эндоваскулярных методов лечения илеокавального сегмента в настоящее время недостаточно, однако у ряда пациентов малоинвазивная методика реконструкции данной локализации может стать полноценной альтернативой открытому хирургическому вмешательству. В настоящей статье представлен клинический случай 48-летнего пациента с тяжёлой посттромботической болезнью, выраженной венозной хромотой, двусторонней илеофеморальной обструкцией и протяжённой окклюзией инфраренального сегмента нижней полой вены. Выполнена одномоментная эндоваскулярная реканализация и реконструкция илеокавально-бедренного сегмента под контролем внутрисосудистого ультразвука с имплантацией венозных стентов Abre™ и Wallstent™. Через 6 мес. отмечено значимое клиническое улучшение: уменьшение симптомов венозной гипертензии, увеличение дистанции ходьбы, снижение выраженности посттромботической болезни по шкале Villalta и улучшение показателей по шкале VCSS (Venous Clinical Severity Score).</p></abstract><trans-abstract xml:lang="en"><p>According to widely published data, intravascular imaging (IVI)-guided endovascular venous stenting is an effective and safe alternative to open reconstructive surgery of the iliofemoral venous segment. Data on the potential of endovascular treatments for the ileocaval segment are currently insufficient. However, for some patients, this minimally invasive reconstructive surgery may be a viable alternative to open surgery. This article presents the case of a 48-year-old patient with severe postthrombotic disease, severe venous claudication, bilateral ileofemoral obstruction, and extensive occlusion of the infrarenal part of the inferior vena cava. Single-step endovascular IVI-guided recanalization and reconstruction of the ileocaval-femoral segment was performed with implantation of Abre™ and Wallstent™ venous stents.
