Оценить динамику функции почек на фоне терапии ингибиторами натрий-глюкозного котранспортера 2 типа (иНГЛТ-2) у пациентов с сочетанием фибрилляции предсердий (ФП) и хронической болезнью почек (ХБП).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Включено 100 пациентов с ФП и ХБП 3-4 стадии. Оценивались уровень креатинина сыворотки, клиренс креатинина (КК) по Кокрофту-Голту, скорость клубочковой фильтрации (СКФ) по формуле CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration). Анализировались снижение СКФ ≥20%, удвоение уровня креатинина и развитие острого повреждения почек (ОПП).</p></sec><sec><title>Результаты</title><p>Результаты. Через 18 мес. в основной группе СКФ в среднем был на 10,1 (95% доверительный интервал: 3,6; 16,7) мл/мин/1,73 м² выше, чем в контрольной группе. У пациентов с ХБП С3В стадии уровень СКФ увеличился на 4,3±10,5 мл/мин/1,73 м² от исходного. У пациентов, принимавших иНГЛТ-2, увеличивалась вероятность перехода из ХБП С3В в ХБП С3А стадии: отношение шансов (OR, odds ratio) =0,53; 95% доверительный интервал: 0,42; 0,67. Разница показателей функции почек наблюдалась только при приеме ингибиторов ренин-ангиотензиновой системы. Не наблюдалось удвоения уровня креатинина и развития острого повреждения почек. По частоте снижения СКФ ≥20% различий между группами не было.</p></sec><sec><title>Заключение</title><p>Заключение. Нефропротективный эффект иНГЛТ-2 сохраняется при сочетании ФП и ХБП, особенно у пациентов с ХБП С3В стадии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the changes of renal function during treatment with sodium-glucose cotransporter-2 (SGLT-2) inhibitors in patients with atrial fibrillation (AF) and chronic kidney disease (CKD).</p></sec><sec><title>Material and methods</title><p>Material and methods. A total of 100 patients with AF and stage 3-4 CKD were included. Serum creatinine levels, creatinine clearance (CrCl) according to the Cockcroft-Gault equation, and glomerular filtration rate (GFR) using the CKD-EPI equation were assessed. A decrease in GFR ≥20%, a doubling of creatinine levels, and the development of acute kidney injury (AKI) were analyzed.</p></sec><sec><title>Results</title><p>Results. After 18 months, the GFR in the study group averaged 10,1 (95% confidence interval: 3,6; 16,7) ml/min/1,73 m2 higher than in the control group. In patients with stage C3B CKD, the GFR level increased by 4,3±10,5 ml/min/1,73 m2 from baseline. Patients taking SGLT-2 inhibitors had an increased risk of progression from stage C3B CKD to stage C3A: odds ratio (OR) =0,53; 95% confidence interval: 0,42; 0,67. Differences in kidney function were observed only with renin-angiotensin system inhibitors. No doubling of creatinine levels or development of acute kidney injury was observed. There were no differences between groups in the incidence of a decrease in GFR ≥20%.</p></sec><sec><title>Conclusion</title><p>Conclusion.
