Определить и охарактеризовать особенности субфракционного спектра липопротеинов высокой плотности (ЛВП) у пациентов с наличием или отсутствием атеросклероза бедренных артерий (БА), а также в зависимости от наличия ожирения.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 97 пациентов 3480 лет (средний возраст 59 лет), с наличием (n=47) или отсутствием (n=50) атеросклероза БА, подтверждённого инструментальными методами. Липидный профиль определяли стандартными биохимическими методами. Субфракционный спектр ЛВП анализировали с помощью электрофореза в 3%-ном полиакриламидном геле с использованием тест-системы Lipoprint HDL Subfractions Testing System (Quantimetrix, США). Дополнительно проведён сравнительный анализ субфракционного спектра ЛВП у пациентов с атеросклерозом БА в зависимости от наличия (n=23) или отсутствия (n=24) ожирения (индекс массы тела ≥30 кг/м2).</p></sec><sec><title>Результаты</title><p>Результаты. Статистически значимых различий в уровнях стандартных показателей липидного профиля между пациентами с атеросклерозом БА и без него не выявлено. Вместе с тем, установлены различия в субфракционном спектре ЛВП в виде более низкой доли крупных субфракций ЛВП1-3 и накоплением мелких ЛВП8-10 у больных с атеросклерозом БА. Согласно результатам многофакторного регрессионного анализа, ассоциация между характером распределения субфракций ЛВП и наличием атеросклероза БА сохранялась после поправки на пол, возраст и наличие ожирения. Соотношение мелкие/крупные субфракции ЛВП статистически значимо ассоциировано с наличием атеросклероза БА и продемонстрировало удовлетворительную дискриминационную способность при ROCанализе. В группе пациентов с атеросклерозом БА ожирение ассоциировалось с дополнительным смещением субфракционного спектра ЛВП в сторону менее атеропротективного профиля.</p></sec><sec><title>Заключение</title><p>Заключение. Атеросклероз БА ассоциирован с более низкой долей крупных частиц ЛВП и накоплением мелких субфракций. Эти особенности носят качественный характер и не выявляются при стандартной оценке показателей липидного профиля, что подчёркивает значимость анализа субфракционного спектра ЛВП. У пациентов с атеросклерозом БА наличие ожирения было сопряжено с дополнительным смещением субфракционного спектра ЛВП в сторону частиц с потенциально меньшей атеропротективной активностью.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To determine and characterize the features of the high-density lipoprotein (HDL) subfractional profile in patients with or without femoral artery (FA) atherosclerosis, as well as in relation to obesity status.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 97 patients aged 34-80 years (mean age 59 years), with (n=47) or without (n=50) FA atherosclerosis, confirmed by imaging methods. Lipid profiles were determined using standard biochemical methods. The HDL subfraction profile was analyzed by electrophoresis in 3% polyacrylamide gel using the Lipoprint HDL Subfractions Testing System (Quantimetrix, USA). Additionally, we analyzed HDL subfractional profile in patients with FA atherosclerosis depending on the presence (n=23) or absence (n=24) of obesity (body mass index ≥30 kg/m2).</p></sec><sec><title>Results</title><p>Results. No significant differences in the levels of standard lipid profile parameters were found between patients with and without FA atherosclerosis. However, differences in the HDL subfractional profile were found in the form of a lower proportion of large HDL1-3 subfractions and the accumulation of small HDL8-10 in patients with FA atherosclerosis. According to multivariate regression analysis, the association between the distribution pattern of HDL subfractions and FA atherosclerosis remained after adjustment for sex, age, and obesity. The small/large HDL subfraction ratio was significantly associated with FA atherosclerosis and demonstrated satisfactory discriminatory power in ROC analysis. In the group of patients with FA atherosclerosis, obesity was associated with an additional shift in the HDL subfractional profile toward a less atheroprotective profile.</p></sec><sec><title>Conclusion</title><p>Conclusion. FA atherosclerosis is associated with a lower proportion of large HDL particles and the accumulation of small subfractions. These features are qualitative in nature and are not detected by standard assessment of lipid profile parameters, which emphasizes the importance of HDL subfractional profile analysis.
