Сравнить отдаленные результаты различных методов реваскуляризации миокарда у больных стабильной ишемической болезнью сердца.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 352 пациента со стабильной ишемической болезнью сердца: средний возраст 57,6±7,7 года, перенесших операцию — коронарное шунтирование (КШ) — I группа (n=183) или чрескожные коронарные вмешательства (ЧКВ) — II группа (n=169) с имплантацией стентов с лекарственным покрытием 1 поколения (Taxus/ Sypher), с высокой приверженностью лечению — медикаментозной терапии в послеоперационном периоде. Каждая группа была разделена на 2 подгруппы: 1 подгруппа — с 1-2-сосудистым поражением коронарных артерий (КА), 2 — со стенозом ствола левой КА (ЛКА) и/или с тяжелым многососудистым поражением КА. Первичные конечные точки: смерть от всех причин, кардиальная смерть, нефатальный инфаркт миокарда, нефатальный инсульт. Вторичная конечная точка: повторная реваскуляризация миокарда или показания к ней. Среднее время наблюдения после операции составило 37,9±6 мес.</p></sec><sec><title>Результаты</title><p>Результаты. В отдаленном периоде различий в выживаемости между группами КШ и ЧКВ не выявлено как у больных с 1-2-сосудистым поражением КА — 95,5% vs 96,8%, так и со стенозом ствола ЛКА и/или многососудистым поражением КА — 95,6% vs 94,1%. При этом наблюдались различия в выживаемости в зависимости от степени коморбидности больного, не зависимо от метода реваскуляризации. Не выявлено различий между I и II группами в отношении кардиальной летальности, наступления нефатального инфаркта миокарда и инсульта. Наличие показаний для повторной реваскуляризации миокарда, было значимо выше в группе ЧКВ только в подгруппах больных с многососудистым поражением КА — свобода от реваскуляризаций — 97,1% vs 88,0%, соответственно, КШ и ЧКВ (p=0,034).</p></sec><sec><title>Заключение</title><p>Заключение. Отдаленная выживаемость после ЧКВ с использованием стентов с лекарственным покрытием 1-го поколения в сравнении с КШ не имеет достоверных различий у больных с различной тяжестью поражения КА и высокой степенью приверженности лечению, но выявляется зависимость от степени коморбидности. Потребность в повторной реваскуляризации миокарда чаще возникает после эндоваскулярных вмешательств у больных со стенозом ствола ЛКА и/или многососудистым поражением КА.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To compare long term results of different methods of myocardial revascularization in patients with stable ischemic heart disease.</p></sec><sec><title>Material and methods</title><p>Material and methods. In the study, 352 patients included, with stable ischemic heart disease: mean age 57,6±7,7 y.o. underwent coronary bypass operation (CBG) — group 1 (n=183) and percutaneous coronary interventions (PCI) — group 2 (n=169) with drug eluting stents implantation (1st generation — Taxus, Cypher), with high treatment adherence — medication treatment in postsurgery period. Each group was selected to 2 subgroups: 1 subgroup with 1-2 vessel disease, 2 subgroup with main left coronary artery stem disease and/or severe multivessel disease. Primary endpoints: all-cause mortality, cardiac mortality, non-fatal myocardial infarction, non-fatal stroke. Secondary endpoint: repeated revascularization or appearance of indications to it. Mean time of follow up after CBG was 37,9±6 months.</p></sec><sec><title>Results</title><p>Results. In long term period there were no differences in survival between groups CBG and PCI neither in 1-2-vessel lesion (95,5% vs. 96,8%), nor with stem stenosis or multivessel disease — 95,6% vs 94,1%. Also, there were differences in survival rate depending on the grade of patient comorbidity, regardless revascularization method. There were no differences in groups 1 and 2 in cardiac mortality, nonfatal events. Emerging of indications for repeated myocardial revasculariztion was significantly higher in subgroups with multivessel disease: free from revascularizations, respectively, 97,1% vs 88,0%, in CBG and PCI (p=0,34).</p></sec><sec><title>Conclusion</title><p>Conclusion. Long term survival after PCI with drug eluting stents of 1st generation in comparison to CBG, does not show significant differences in patients with different severity of coronary arteries lesion and high adherence to treatment. However there is dependence on comorbidity evel.
