Изучить наличие и характер взаимосвязи между вариабельностью гликемии (ВГ) и показателями сосудистой стенки, хронического  воспаления,  окислительного  стресса, биологией  теломер у больных сахарным диабетом 2 типа (СД-2).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В одномоментное исследование были включены 50 пациентов с СД-2 без клинических проявлений сердечнососудистых заболеваний  (ССЗ). У  всех участников исследования оценивали: углеводный обмен, вариабельность гликемии; выполняли дуплексное  сканирование  сонных артерий  с  определением толщины комплекса интима-медиа  (ТКИМ), а также наличия атеросклеротических бляшек; измеряли каротидно-феморальную  скорость  распространения пульсовой волны; оценивали  эндотелийзависимую вазодилатацию;  измеряли  длину теломер  и активность теломеразы  (АТ).</p></sec><sec><title>Результаты</title><p>Результаты. Параметры сосудистой стенки, наряду с классическими факторами риска ССЗ, независимо связаны с различными показателями углеводного обмена: гликированным гемоглобином, глюкозой плазмы натощак, индексом HOMA-ИР, С-пептидом, иммунореактивным инсулином. Показатель ВГ средней амплитуды колебаний гликемии (MAGE) связан с повышением ТКИМ у больных СД-2. Выявлена достоверная  связь длины теломер  (ДТ)  не только с хронической гипергликемией, но и с колебаниями глюкозы, а именно, с показателями ВГ стандартное отклонение (SD), MAGE, показатель непрерывного частичного перекрывающегося изменения гликемии (CONGA), и эта связь становится больше при анализе “самых длинных” теломер.  Связь ДТ  с ВГ не зависит  от факторов  риска ССЗ, хронического воспаления и АТ, но зависит от глюкозы плазмы натощак. Не получено достоверной  связи  АТ с показателями  ВГ при СД-2. Установлена прямая корреляционная связь между показателем  ВГ CONGA и маркером  окислительного стресса  малонового диальдегида и независимая обратная связь ВГ (MAGE, SD, CONGA) с ДТ.</p></sec><sec><title>Заключение</title><p>Заключение. ВГ связана  с субклиническим атеросклерозом (ТКИМ) и более “короткой” ДТ  у пациентов с СД-2, что может быть обусловлено активацией окислительного стресса  в условиях колебаний гликемии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate  on  the  presence and  relation characteristics of glycemia  variability (GV) and   vascular   wall  parameters,  chronic inflammation, oxidation status,   telomere  biology in type  2  diabetes patients (DM2).</p></sec><sec><title>Material and methods</title><p>Material and methods. Into the single-movement study, 50 DM2 patients included, with no signs of cardiovascular diseases. All participants  were evaluated on the carbohydrate  metabolism, glycemia variablity, underwent duplex  scan   of  carotid   arteries   with  intima-media  complex  (IMC) measurement, and  assessment for atherosclerotic  plaques;  underwent carotid-femoral pulse wave velocity measurement, endothelium dependent vasodialtion, telomere length measurement and telomerase activity (TA).</p></sec><sec><title>Results</title><p>Results. The parameters of vessel wall, together with the classical CVD risk  factors,   independently   related   with the  various  carbohydrate metabolism parameters: glycated hemoglobin, fasting glucose plasma, HOMA-IR index, C-peptide, immune reactive insulin. The GV of moderate amplitude  of glycemia (MAGE) was related  to IMC increase  in DM2 patients.  There was significant relation of telomere length (TL) not just with a chronic hyperglycemia, but with glucose level fluctuations as well, e.g. with GV standard deviation (SD), MAGE, CONGA (continuous overall net glycemic action), and such relation is higher in the analysis of “the longest”  telomeres.  Relation of TL  and  GV  is not dependent on the cardiovascular risk factors,  chronic inflammation and TA, but depends on fasting plasma glucose.  There was no significant relation of TA with GV in DM2. There was direct correlation of GV CONGA  and oxidative stress marker malonic dialdehyde, and independent negative correlation of GV (MAGE, SD, CONGA) with TL.</p></sec><sec><title>Conclusion</title><p>Conclusion.
