Однако эта  популяция недостаточно  представлена  в клинических исследованиях, вследствие чего ведение таких пациентов представляет собой значительный пробел доказательной  медицины. Острый коронарный синдром (ОКС) предполагает активные лечебные мероприятия с первых мин развития  ишемии миокарда.  Соотношение риска и пользы инвазивного подхода у очень пожилых людей остается неясным и недоказанным,  т.к. не учитываются риски, связанные с индивидуальными особенностями пациента, сопутствующими заболеваниями и старческой астенией (“хрупкостью”).</p><p>В настоящей  статье  приведены  результаты  рандомизированных клинических исследований и регистров последних лет, целенаправленно включивших пациентов с ОКС в возрасте  ≥75 лет. Отмечены особенности    проведения    интервенционного   вмешательства, эффективность  и осложнения  реперфузионной  терапии  при ОКС с подъемом сегмента ST (ОКС↑ST) у лиц старческого возраста. Подробно рассмотрена проблема выбора тактики лечения при ОКС без  подъема  сегмента  ST (ОКС↓ST) у этой категории пациентов: соотношение  рациональности  ранней  инвазивной  стратегии  — коронарографии,  при показаниях, чрескожного коронарного вмешательства  в течение  72 ч, и изначальной  консервативной  терапии — коронарография  и реваскуляризация только при рецидиве ишемии; безопасность  первичного инвазивного подхода; проведение интервенционного вмешательства у всех пациентов старческого возраста  либо предпочтение индивидуального подхода в решении этого вопроса и влияние такого выбора на 5-летний риск сердечно-сосудистой смерти и инфаркта миокарда. Особое внимание уделено необходимости гериатрической  оценки пациентов,  наличию и выраженности старческой астении (“хрупкости”) и ее влиянию на исходы лечения ОКС, качество жизни пожилого пациента после перенесенного острого коронарного события, организации Российским геронтологическим  научно-клиническим центром, Российской ассоциацией  геронтологов и гериатров и Российским кардиологическим  обществом  регистра  ОКС у пациентов ≥80 лет в различных регионах РФ.</p></abstract><trans-abstract xml:lang="en"><p>Cardiovascular diseases impact at most the morbidity, mortality and life quality of the elderly. However this population is underrepresented in clinical trials, hence  management of this sort of patients  is a gap  in evidence.  Acute coronary  syndrome  (ACS) requires  active treatment from the very first minutes from myocardial ischemia onset.  The riskbenefit ratio of invasive approach in a very old is still foggy, as there is no consideration of the risks related to frailty and other individual specifics of a patient, as the comorbidities.</p><p>In the article, randomized trials data is presented, as of registries, that included  aimfully ACS patients  75 years  and  older.  The specifics  of intervention is pointed in, its efficacy, and complications of reperfusion therapy in ACS with ST elevation (STEMI) in the senile.</p><p>In details, an issue considered  on the tactics of ACS treatment in nonST-elevation  ACS (NSTEMI), as  the  rationale  for  earlier  invasive tactics  — coronary  arteriography  and,  if  indicated,  percutaneous coronary intervention during 72 hours, or the conservative therapy at first, and coronary arteriography and revascularization only if ischemia recur; safety of the primary invasive approach;  interventional approach  by default or individualized approach  and influence of such choice on 5-year cardiovascular mortality and myocardial infarction.
