Изучить структурно­функциональные изменения легких у больных инфарктом миокарда с подъемом сегмента ST (ИМ↑ST) с наличием и отсутствием хронической обструктивной болезни легких (ХОБЛ), а также их связь с миокардиальной дисфункцией и системным воспалением.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Включены 189 пациентов с ИМ↑ST: 1 группа — пациенты с ИМ↑ST + ХОБЛ средней и легкой степеней тяжести, 2 — пациенты с ИМ↑ST без патологии легких. Группы были сопоставимы по клинико­анамнестическим характеристикам. Исследование функции легких и забор крови выполнялись на 10­12 сут. ИМ↑ST. Для сравнения показателей, отражающих структурнофункциональные изменения легких и сопоставления концентрации С­реактивного белка (СРБ) и N­терминального фрагмента мозгового натрийуретического пептида (NT­proBNP) сформирована группа контроля из лиц без патологии легких, которая была сопоставима по полу и возрасту с больными ИМ↑ST.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с ХОБЛ выявлены более высокие значения показателей, характеризующие долю остаточных объемов в структуре легких. Более высокий уровень остаточного объема (RV) отмечен также у больных ИМ↑ST ­ ХОБЛ по сравнению с группой контроля, однако отношение RV/общая емкость легких (TLC) не превысило нормативных значений. В обеих группах оказались меньшие значения диффузионной способности легких (DLCO) по сравнению с группой контроля. Наименьшие значения DLCO выявлены у пациентов + ХОБЛ. Концентрация NT­proBNP (Н=41,6; р&lt;0,001) и СРБ (Н=38,6; р&lt;0,001) у пациентов с ХОБЛ значимо превышала концентрацию этих маркеров у больных ИМ↑ST без ХОБЛ и лиц группы контроля. Выявлены отрицательные корреляционные связи концентраций NT­proBNP и СРБ с объемом форсированного выдоха за 1 сек, модифицированным индексом Тиффно, DLCO и положительные с показателями внутригрудного объема, RV/TLC.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с ИМ↑ST выявлено увеличение доли остаточных объемов в структуре легких. В большей степени уровень остаточных объемов повышен у пациентов с ИМ↑ST + ХОБЛ. Обнаружены ассоциации NT­proBNP и СРБ со структурно­функциональными параметрами легких вне зависимости от наличия ХОБЛ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the structural and functional changes in the lungs of ST elevation myocardial infarction (STEMI) patients with absence or presence of chronic obstructive lung disease (COPD), and the relation with myocardial dysfunction and systemic inflammation.</p></sec><sec><title>Material and methods</title><p>Material and methods. Totally, 189 STEMI patients included: group 1 — STEMI with COPD of moderate and mild grade, 2 — STEMI with no lung pathology. Groups were comparable by clinical and anamnestic parameters. Assessment of lung function and blood collection were done at 10­12 day of STEMI. For comparison of the parameters representing structural and functional changes in the lungs and comparison of C­reactive protein (CRP), N­terminal pro­brain natriuretic peptide (NT­proBNP) concentration, a control group was formed with no pulmonary pathology, comparable by age and sex with the STEMI patients.</p></sec><sec><title>Results</title><p>Results. In COPD patients, higher values revealed of the parameters representing the part of residual volumes in pulmonary structure. Higher residual volume (RV) was found also in STEMI and no COPD comparing to controls, however the relation RV/TLC (total lung capacity) was not higher than normal range. In both groups there were lower values of diffusion lung capacity (DLCO) comparing to controls. The lowest DLCO found in COPD patients. Concentration of NT­proBNP (H=41,6; p&lt;0,001) and CRP (H=38,6; p&lt;0,001) in COPD was significantly higher in STEMI with no COPD patients than in controls. The negative correlations found for NT­proBNP and CRP with forced expiratory volume 1 sec, FEV/FVC1, DLCO, and positive — with the values of thoracic volume, RV/TLC.</p></sec><sec><title>Conclusion</title><p>Conclusion. In STEMI patients the increase revealed of residual lung volumes. Mostly the level of residual volumes is high in STEMI and COPD patients.
