Одним из широко обсуждаемых феноменов, возникающих при анализе ассоциации ожирения с сердечно-сосудистыми заболеваниями, их осложнениями и смертностью, является феномен “гетерогенности фенотипов ожирения в отношении сердечно-сосудистого риска”. В настоящее время выделено два основных фенотипа — метаболически здоровое и метаболически нездоровое ожирение, первый из которых характеризует пациентов с избыточным весом и ожирением в отсутствии ассоциированных метаболических нарушений. Вместе с тем сегодня широко обсуждается не только целесообразность выделения, но и существование этого фенотипа как такового. В статье приводится обзор существующих подходов к верификации метаболических фенотипов ожирения и информации, касающейся вопросов эпидемиологии и ассоциации метаболически здорового ожирения с риском сердечно-сосудистых заболеваний и смерти, а также обсуждаются факторы, определяющие характер фенотипа ожирения.</p></abstract><trans-abstract xml:lang="en"><p>Obesity is associated with numerous metabolic disorders, the development and progression of a wide range of conditions and diseases, primarily cardiovascular disease and type 2 diabetes. One of the widely discussed phenomena of the association of obesity with cardiovascular diseases, their complications and mortality is the phenomenon of “heterogeneity of obesity phenotypes in relation to cardiovascular risk”. Currently, two main phenotypes have been identified — metabolically healthy and metabolically unhealthy obesity. At the same time, not only the expediency of separation, but also the existence of this phenotype is widely discussed today.
