Сопоставить известные методы изучения артериального давления (АД) в утренние часы и новую методику оценки среднего утреннего уровня АД за фиксированные временные промежутки (ВП), сравнив их воспроизводимость и валидность.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проанализирована база данных, содержащая 983 суточных мониторирования АД (СМАД). Анализировали результаты СМАД у больных АГ без серьезных сопутствующих заболеваний на фоне недельной отмены антигипертензивной терапии. Оценивались уровни АД за сут., за день и за ночь, величина и скорость утреннего подъема АД; был предложен новый метод оценки среднего утреннего уровня АД за фиксированные ВП, основанный на расчете среднего уровня АД за следующие ВП: с 5:00 до 7:00 ч, c 7:00 до 9:00 ч, с 9:00 до 11:00 ч. Для выделения границ ВП была использована кривая скорости нарастания/убывания уровня АД в течение сут. Для оценки воспроизводимости нового метода рассчитывались корреляционные связи между первой и второй группами показателей, рассчитанных по новой методике на основании двух повторных СМАД, проведенных с интервалом 1-2 нед. (n=90). Для изучения валидности нового метода оценки среднего утреннего уровня АД использовался объективный критерий валидизации, индекс массы миокарда левого желудочка (ИММЛЖ), с которым были рассчитаны корреляционные связи для показателей, полученных при помощи новой методики. Были выполнены 98 эхокардиографических исследований на аппарате Acuson 128XP. ИММЛЖ рассчитывался по формуле L. Teichholtz, 1976г.</p></sec><sec><title>Результаты</title><p>Результаты. Новый метод оценки среднего утреннего уровня АД обладал хорошей воспроизводимостью в то время как у динамических показателей СМАД (скорость утреннего подъема и величина утреннего подъема) воспроизводимость была низкой. Новая методика оценки утреннего АД характеризовалась высоким уровнем валидности: показатели, полученные при помощи этой методики достоверно коррелировали с критериями валидизации: ИММЛЖ и средним дневным уровнем АД.</p></sec><sec><title>Заключение</title><p>Заключение. Предложенная новая методика оценки среднего утреннего уровня АД обладала высокими валидностью и воспроизводимостью. Из трех предложенных новой методикой ВП для оценки утренних АД, наиболее оптимальным представляется ВП с 7 до 9 ч. В этот период отмечалась максимальная воспроизводимость показателей, полученных по этой методике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To compare the known methods of blood pressure (BP) measurement in the morning and a new method for estimating the average morning BP for fixed time intervals, comparing their reproducibility and validity.</p></sec><sec><title>Material and methods</title><p>Material and methods. We analyzed database containing 983 cases of ambulatory blood pressure monitoring (ABPM). Results of ABMP in patients with hypertension without severe associated diseases and weekly cancellation of antihypertensive therapy were analyzed. Blood pressure data per 24-hour period, day and night, level and rate of morning BP surge were assessed. In addition, we proposed new method for estimating the average morning BP level for fixed time intervals, based on calculating the average BP level for the following time windows: from 5:00 to 7:00 a.m., from 7:00 to 9:00 a.m., from 9:00 to 11:00 a.m. To assess the reproducibility of the new method, we determined correlations between the first and second group results calculated by the new method based on two repeated ABPM conducted with 1-2 weeks interval (n=90). To study the validity of the new method for measurement of average morning BP level, we used an objective validation criterion — Left Ventricular Myocardium Mass Index (LVMMI). There were performed 98 echocardiographic tests with the use of Acuson 128XP. LVMMI was calculated using the L. Teichholtz method (1976).</p></sec><sec><title>Results</title><p>Results. The new method for measurement of average morning BP level had good reproducibility while the reproducibility of dynamic showings of ABMP (level and rate of morning BP surge) was low. A new method for assessing morning blood pressure was characterized by a high level of validity: the indicators obtained using this technique reliably correlated with the validation criteria: LVMI and the average daily blood pressure level.</p></sec><sec><title>Conclusion</title><p>Conclusion. The proposed new method for estimating the average morning level of blood pressure has a high level of validity and reproducibility. Time interval from 7 to 9 a. m. seems to be the most optimal.
