Сравнение эффектов лерканидипина и рамиприла на скорость экскреции альбумина у пациентов с артериальной гипертензией, сахарным диабетом 2 типа и микроальбуминурией: исследование DIAL (Diabete, Ipertensione, Albuminuria, Lercanidipina)

Цель. Микроальбуминурия (МАУ) и гипертензия (АГ) являются факторами риска (ФР), развития диабетической нефропатии у пациентов с сахарным диабетом 2 типа (СД-2). Согласно последним данным, блокада ренин-ангиотензиновой системы (РАС) замедляет развитие диабетической нефропатии. Однако информация о нефропротективном действии блокаторов кальциевых каналов (АК) остается противоречивой. Была дана оценка эффективности лерканидипина в сравнении с рамиприлом в отношении снижения скорости экскреции альбумина (СЭА) и уровня артериального давления (АД) у пациентов с мягкой и умеренной АГ, СД-2 и персистирующей МАУ. Материал и методы. В многоцентровое, рандомизированное, двойное слепое исследование с использованием активного контроля в параллельных группах (гр.) включены 277 больных; 180 были рандомизированы в гр. лерканидипина (10-20 мг/сут.) либо рамиприла (5-10 мг/сут.); период наблюдения составил 9-12 мес. Основной конечной точкой было изменение СЭА по сравнению с исходным уровнем. Результаты. Через 9-12 мес. наблюдения, снижение СЭА в гр. лерканидипина и рамиприла составило -17,4±65 μг/мин (р<0,05) и -19,7±52,5 μг/мин (р<0,05), соответственно, без достоверных различий между гр. Статистически значимое снижение уровней систолического и диастолического АД наблюдалось в обеих гр. лечения (для каждой гр. р<0,0001). Заключение. Терапия лерканидипином (10-20 мг/сут.) не усиливает альбуминурию у больных АГ, СД-2 и МАУ. Как лерканидипин, так и рамиприл достоверно снижали СЭА, при отсутствии статистически значимых различий между гр. лечения.
